You will have the opportunity to make a positive impact on the lives of our patients by ensuring their care needs are met effectively and efficiently.Schedule: M - F 8:00am - 5pm (Some on-call requirements)Office Address: 718 S State St, Clarks Summit, PA 18411Benefits and Perks...Medical, Dental & Vision Insurance401(k) with a 6% matchPaid Time Off10 Paid Holidays Employee Assistance ProgramEmployee Discounts (retail, hotel, food, restaurants, car rental, and much more!)Voluntary Term Life and AD&D InsuranceLegal Services InsuranceShort-Term and Long-Term DisabilityAccident, Critical Illness & Hospital Indemnity InsuranceYou will...Serves as the mentor to other Care Coordinators, offering guidance and support to enhance their performanceLeads and facilitate regular team meetings, fostering a collaborative and high-performance work environment.Participates in strategic planning initiatives related to caregiver coordination and patient care, offering insights and recommendations based on experience and expertise.Identifies and implements solutions to enhance the efficiency and effectiveness of care coordination processes.Leads the development and implementation of new policies, procedures, and best practices to improve operational efficiency and ensuring compliance with regulatory requirements.Collaborates with senior management to develop and implement strategic plans and initiatives that align with organizational goals.Designs and conducts training programs for Care Coordinators, ensuring they are well-equipped to handle complex inquiries and tasks.Evaluates and refines training materials and programs with stakeholders to ensure they meet the evolving needs of the team and organization.Main point of contact for caregivers, addressing complex inquiries, providing support, and ensuring their needs are met.Coordinates caregiver schedules, ensuring adequate coverage for all clients while considering caregiver availability and preferences.Conducts regular check-ins with caregivers to assess their needs, address concerns, and provide guidance.Schedules client visits based on their needs and caregiver availability.Adapts schedules as needed to accommodate changes in client conditions, caregiver availability, or other unforeseen circumstances.Utilizes scheduling software and tools to optimize efficiency and ensure accuracy.Oversees the implementation and utilization of EVV systems for accurate tracking of caregiver visits, ensure compliance with regulatory requirements and address any discrepancies.Maintains up-to-date knowledge of relevant regulations, policies, and guidelines related to home healthcare services.Ensures adherence to regulatory requirements and company policies through diligent tracking and monitoring.Collaborates with management to address any compliance issues and implement corrective actions, as necessary.Tracks caregiver hours worked, time-off requests and maintain accurate records of their accrued leave to ensure timely and accurate payroll processing.Tracks caregiver participation in in-service training and maintain training records.Handles workers' compensation incident reporting process, ensuring timely and accurate submission of reports.Collaborate with insurance providers and legal counsel as needed to resolve workers' compensation claims.Maintains organized and up-to-date records of client and caregiver information, including schedules, contracts, and medical documentation.Ensures compliance with document retention policies and regulatory requirements.Participate in other projects or duties as assigned.
