This role provides a unique opportunity to support women through high-risk pregnancies and related health challenges. The ideal candidate will have experience in women's health and a commitment to delivering high-quality, patient-centered care. You will play a crucial role in offering emotional support, connecting patients with resources, and working closely with healthcare providers to ensure the best outcomes for our patients.These duties are to be performed in a highly confidential manner, following the mission, values, and behaviors of Insight Hospital and Medical Center. Employees are further expected to provide a high quality of care, service, and kindness toward all patients, staff, physicians, volunteers, and guests.DESCRIPTION:Duties and Responsibilities: Conduct comprehensive assessments to identify the social and emotional needs of patients with high-risk pregnancies. Develop and execute personalized care plans in collaboration with the healthcare team. Deliver individual, group, and family counseling to support patients through their pregnancy journey. Link patients to community resources, social services, and support networks tailored to their needs. Work closely with obstetricians, nurses, and other healthcare professionals to ensure integrated and effective care. Educate patients and their families about available resources and coping strategies for managing high-risk pregnancies. Maintain thorough and confidential records of patient interactions, assessments, and care plans. Contribute to case management meetings and support program development and enhancement. Stay current with best practices and emerging trends in women's health and social work.QUALIFICATIONS: Required Knowledge, Skills, and Abilities: Master's degree in Social Work (MSW) from an accredited program. Current Illinois licensure as a Clinical Social Worker (LCSW) or equivalent. Proven experience and deep passion for working in women's health, especially with high-risk pregnancies. Exceptional interpersonal and communication skills, with the ability to build trust and effectively engage with patients and families. Strong organizational abilities and a capacity to manage a diverse caseload. Proficiency with electronic health record (EHR) systems. Dedication to patient-centered care and promoting health equity. Ability to collaborate effectively with a multidisciplinary team and work independently when necessary. Preferred Qualifications: Previous experience in a clinical setting focusing on women's health or high-risk pregnancies. Bilingual proficiency is a plus.
