DescriptionHomeward Bound Nurses are responsible for safely and professionally providing exceptional nursing care and life enrichment services to the people we serve. Nurses are also responsible for leading others through training, coaching and mentoring frontline Nurses and Direct Care Staff. Homeward Bound LPNs work with the same four residents, making it the perfect position for nurses looking to build relationships with those they serve.We have a variety of shifts available including:Full Time & Part Time Days (6:30am-2:30pm)Full Time & Part Time Afternoons (2:00pm-10:00pm)Full Time & Part Time Overnights (9:30pm-7:00am)Essential Job Duties:Nursing Practice Medications and Treatments Administer medication and treatments as ordered by a doctor. Enter new medication orders into appropriate eMAR (Therap or ACS) immediately. Complete narcotic counts at shift change. Document counts in eMAR and narcotic book. Follow dual signature protocol. Assist with inventory management and ordering supplies for individuals as needed. Assess and monitor each individual's health situation. Check Bluestone Bridge every shift, multiple times per shift. Complete head to toe assessments on assigned individuals once per shift. Follow up with all individual health issues as needed. Thoroughly complete charting, MAR's, progress notes and other required documentation timely and accurately (before clocking out). Ensure there is no missing documentation. Communicate regularly and timely with co-workers, supervisors, Nurse Case Managers, and family members regarding individual health status updates and progress. Provide and receive timely and thorough shift reports. Handle health emergency situations and give direction to others. Communicate and educate unlicensed staff regarding individual medical conditions, medications and treatments. Maintain high level of confidentiality and compliance with HIPAA. Appointments Fill out Referral Sheets prior to appointments. Transport and accompany individuals to appointments. Ensure appointments are attended as scheduled. In the rare occurrence that an appointment must be cancelled, reschedule it immediately. Life Enrichment Program Implementation Seek to understand each individual, their goals, dreams and interests, and support them in living their best life! Read, comprehend and implement individualized program plans. Participate in and implement Person Centered Planning activities. Participate in providing high quality personal care and program services to individuals served. Promote Choice and Personal Interests Actively support individuals in making their own choices, directing their own care as much as possible, and pursuing their interests. Offer or suggest new things on occasion. Ensure individuals go on fun and interesting outings frequently. Suggest options for outings that may be of interest to individuals. Assist them in selecting and planning outings. Encourage and facilitate community inclusion for each individual. Community inclusion can take the form of friendships, clubs, employment, teams, groups, etc. To be "included" suggests when someone is gone they will be missed by others. Transport and accompany individuals to all appointments and outings. Support individuals with observing their personal religious beliefs and practices such as accompanying them to church and assisting them in celebrating religious holidays. Daily Living Actively engage in conversation and activities with individuals at all times. Encourage individuals to participate to the fullest extent possible in completing activities of daily living including meal preparation, bathing, toileting, personal hygiene, dressing, eating, communicating, Passive Range of Motion, transporting, mobility and involvement in activities at home and in the community. Complete and record accurate data on individual programs, goals and activities. Complete appropriate documentation and follow through on employee incidents, individual incidents, HBI incidents and Vulnerable AdultChild Protection issues. Ensure a safe and sanitary living environment for the individuals, including but not limited to household cleaning, laundry, shoveling snow and salting sidewalksdriveway. Participate in building a pleasant, fun and happy home life and physical environment for the individuals. Decorate the home with styles and items that are of interest to the residents. Celebrate birthdays, events, milestones, achievements, seasons, and holidays in the home. Professionalism Communication Engage only in effective and respectful interpersonal communication with all HBI personnel, individuals, and external customers while maintaining a calm and professional demeanor. Receive and accept professional feedback and criticism to improve service quality and job performance. Answer the house phone promptly and professionally. Return any missed calls immediately. Check HBI e-mail once per shift to stay updated on new and important information. Maintain a high level of confidentiality. Training and Education Attend and receive all scheduled and required training. Perform job duties in accordance with HBI training, policies and procedures. Actively seek training and education to continue developing nursing and professional skills. Teamwork Respectfully cooperate with all HBI personnel, management, individuals, families, guardians and external customers to ensure excellent service delivery. Accept direction and instructions from HBI supervisors and management. Build and maintain high levels of integrity and trustworthiness with all HBI personnel, management, individuals, families, guardians and external customers. Assume additional responsibilities as needs arise to ensure excellent service delivery. Report to work on time, take appropriate breaks, and strive for Perfect Attendance. Respond to necessary schedule fluctuations, and organize workload to assure completion of tasks in a timely manner. Apply best practices for allocating HBI resources (eliminate wasteful practices). Comply with organizational and regulatory policies, procedures, and standards.Safety Work safely, performing job tasks as trained and asking for proper instruction when in doubt. Abide by safety rules. Immediately report all accidents, near misses, injuries or illnesses to the supervisor, and if possible, help in filling out an incident report form, First Report of Injury, and any other documentation as needed. Identify, resolve and report hazardous conditions and unsafe behaviors to prevent injuries. Immediately, safely and respectfully stop unsafe behavior that could lead to employee or resident injury. Coach and train co-workers on safe behavior. Notify supervisor of hazardous conditions and unsafe behavior, as needed. Act immediately to remove or resolve workplace hazards (i.e. fixremove trip hazards, clear icesnow on sidewalksdriveway, report issuesconcerns to supervisor or OA on call). Engage and participate in fire and weather safety drills per HBI policy. Use appropriate personal protective equipment (PPE) when required. Cooperate with Homeward Bound management on accident investigations, workplace inspections, and participate in safety training. Leadership Training Provide initial and ongoing training of employees as assigned. Train, direct and support the individuals through implementation of individualized program plans. Developing and providing training to HBI Nurses on specific topics is recommended. Participation in a Homeward Bound committee is recommended. Collecting data and conducting audits and analysis on Homeward Bound health and safety practices is recommended. Coaching Provide immediate and ongoing positive and critical performance feedback to employees. Follow up with supervisor as needed. Direct and coordinate others in the completion of necessary work activities to ensure excellent service delivery. Monitor employees as assigned, provide on the job training, mandatory training and documentation. RequirementsCurrent nursing license in the state of MN.A valid Minnesota driver's license with a good driving record is mandatory.Must demonstrate basic computer skills.You must be able to lift a minimum of 60 pounds as most of our individuals require assistance with transfers.You must be able to understand, speak, read, and write the English language.Because we provide services to vulnerable children and adults Homeward Bound will only employ applicants who pass a criminal background check.Ability to timely and accurately type documentation in online database.Ability to type at, or above, a minimum of 20 words per minute.
