Job DetailsJob LocationBronx, NYPosition TypeFull TimeSalary Range$70,000.00 - $70,000.00 SalaryyearDescriptionPOSITION: Assistant Director of Social Services - Jasmine CourtREPORTS TO: Director of Social ServicesCLASSIFICATION: Full-Time - ManagementEXEMPTNONEXEMPT: ExemptGENERAL DESCRIPTION:LSSNY's 24-hour supportive housing program at Jasmine Court serves extremely vulnerable individuals, including people with histories of homelessness and mental illness. The Assistant Program Director promotes participant wellness and autonomy through advocacy, assessment, planning, communication, education, resource management, and service facilitation, under the guidance of the Director of Social Services. The Assistant Director of Social Services is classified as an essential worker, and as such, accepts responsibility for the completion of their critical services to participants on site, even during times of emergency.These include,but are not limited to, emergencies causedbyweather, nationwide pandemics, civil unrest, and domestic threats.MAJOR DUTIES AND RESPONSIBLITIES:Support and advocate for participants on the High-Risk caseload (total # to be determined).Establish and maintain therapeutic relationships with participants, maintaining professional boundaries and a non-judgmental, unbiased approach.Conduct regular meetings with participants to provide support, advocacy and other case management services. Conduct staff meetings in rotation with the Director of Social Services.Review and complete psychosocial assessments and complete SPOA referrals as needed.Participate in individualized service plans as well as assists Director of Social Services with review.Assist case managers with linking participants with appropriate providers and resources throughout the continuum of health and human services and care settings based on the needs and values of participants. Escort participants to scheduled and unscheduled psychiatricmedical care. Ensure that participants receive safe, effective, client-centered, timely, and equitable care.Ensure at least weekly contacts per month with each participant on Geriatric High Risk case load.Support Director of Social Services in monitoring group work.Support Director of Social Services with onboarding and training new staff.Ensure thesafehandling, service and delivery of meals to all participants,in accordance with theNYC Food Safety Guidelines.Assist with reviewing monthly unit inspections for each participant, ensure proper follow-up to conjunction with the Director of Social Services and in relationship to the weekly staff meeting, will develop a list of High-Risk participants in need of weekend support.Conduct intake interviews for new participants and assist Director of Social Services with the intake process.Ensures that participant records are maintained by the staff as "audit-ready" at all times. This includes, but is not limited to:Psychosocial Assessments ReviewService Plans ReviewProgress Notes ReviewUpdated medical and mental health information.Perform duties of the Director in their absence.Adhere toconfidentiality protocols outlined by the agency.Perform other duties as assigned by the Director of Social Services.Represent Lutheran Social Service of New York's mission, values, ethics, and uphold agency Code of Conduct, at all times.On call 24 hoursday, 7 daysweek.SUPERVISES:Case ManagersQualificationsEDUCATION:Master's degree in social work or related field preferred, orBachelor's degree in social work or related field and must be actively enrolled in a master's program in a similar field (to be obtained within a year from hire)EXPERIENCE:Minimum of 2 years in the field of human servicesWorking knowledge of issues related to homelessness, HIV, substance use, mental illness, and entitlement programs. Familiarity with housing first model.SKILLS & ABILITIES:Able to establish and maintain therapeutic relationships with participants and positive working relationships within LSSNY and with external agencies.Sensitive to intersectionality, different cultures, housing equality and a demonstrated ability to work within a socialracial justice framework.Excellent spoken and written expression.Strong time management skills, able to handle multiple tasks and assignments at once.Crisis intervention skills.Willing to participate in continuing education.Flexible, able to work some early or late day schedules, holidays and weekends.Proficient in internet navigation, Microsoft Office 365.DATE REVISED: 242025
