I. POSITION SUMMARYThe Enhanced Care Management (ECM) Director will oversee ECM program development, implementation, and operations. In accordance with CalAIM, Directror will build ECM program capacity and infrastructure. Will oversee ECM staff to provide efficient, comprehensive, and effective ECM care management services to older adults and disabled members of managed health care plans. II. PRIMARY RESPONSIBILITIESOversees ECM Program implementation and design processes, including data system development. Manages effectiveness of service delivery through on-going supervision to all ECM team members.Ongoing monitoring of client care coordination documentation including assessments, care plans, community support referrals, outcome measures and progress notes.Ensures timely and high-quality documentation of services by ECM in accordance with all agency and contractual requirements.Train, monitor, and support ECM staff to provide appropriate ECM services to managed care members, ensure on-going achievement of quality measures.Ensure culturally appropriate and timely in-person ECM activities.Maintains ongoing communication with managed care providers (MCP) and other contractual partners.Ongoing communication and collaboration with MCP, provide updates on program efforts. Ensure timely outreach to eligible members.Responsible for timely ECM reporting, billing and effectiveness of data management systems.Ensure accurate data input in compliance with MCP standards and requirements.III. SUPERVISORIAL RESPONSIBILITIESECM Director will oversee interviews, hiring, onboarding, and training of ECM team members. Work with organization as necessary to carry out supervisorial responsibilities.Provide weekly supervision to all ECM team members. IV. POSITION QUALIFICATIONSA. Educational RequirementsMaster's degree in social work, Gerontology, Health Administration, Human Services, or related field required. Master's degree desirable.Active licensure, LCSW LVN 2 years supervisorial experienceMinimum three years of experience working with older adults.Fluency in English (bilingual in Spanish strongly preferred).Previous experience working with an ethnically and economically diverse population.Previous care management experience strongly preferredPatient Coordination and Healthcare experience strongly preferredB. Other RequirementsMust have reliable transportation, current driver's license, clean driving record, and proof of auto insurance.Travel is required within Los Angeles County.Must pass a criminal background check.V.
