Full job descriptionThis position is in person. No remote work available.Vitability Health is seeking an experienced and organized Facility Care Manager to join our team. The care manager will be responsible for conducting initial and updated comprehensive assesments and coordinating daily operations. The ideal candidate should have excellent communication skills, a strong attention to detail, and the ability to work in a fast-paced environment.Vitability Health is leading the change in how providers deliver remote care. We are a team of dedicated, mission-driven pros who seek to work with colleagues who inspire us and share our mission and core valuesQualificationsExperience with elderly patients strongly reccomended (MACNALPNRNs strongly encouraged to apply)Technically savvy and comfortable using tools such as iPads for chartingInterest in operating in a new, exciting clinical program and become an integral player in the development of processes and best practices in caring for RPM patientsStrong verbal and written communication skillsValid driver's license with a clean driving recordAbility to lift and carry equipment up to 20 poundsComfortable seeing patients and delivering a positive experienceFull COVID-19 vaccination is a requirement for this position. Medical and religious exemptions may be granted with proper documentation.You will adhere to all federal, state, and local regulations as well as all client requirements and will obtain necessary proof of vaccination prior to employment to ensure compliance.ResponsibilitiesConduct initial and updated comprehensive assesmentsMaintain patient medical records and documentation consistent with state regulationAs a care team member, you love utilizing motivational communication techniques and strategies, to help drive positive health behavior change and improved patient outcomesThe MACNALPNRN plays a vital role in educating patients on how to manage their acute or chronic medical conditions, with a goal to maintain health, wellness, and to avoid emergency room utilization and hospitalizationSignificantly impact longitudinal patient engagement in RPM program(s)Provide preventive health and disease management education and coach
