Youth ACT Case Manager IIFLSA Status: Full-time, Salary, ExemptFTE: 1.0Salary Range: $55,000 - $60,000annuallyWebsite: is hiring a Mental Health Case Manager to support our Youth Assertive Community Treatment (ACT) Teams. This position will serve clients in Ramsey, Dakota and Hennepin Counties. This position is responsible to assist individuals who have serious and persistent mental illnesses andor chemical dependency to live self-directed and satisfying lives by providing services directly, minimally using outside resources and diverting hospitalizations by finding alternative interventions whenever possible. When there are other providers, there is close coordination and collaboration with these other outside resources to ensure the best possible service.Primary ResponsibilitiesBuild trusting relationships with clients to encourage their active participation in treatment and recovery.Assist clients in managing their mental health symptoms, daily living skills, and overall well-being.Ensure timely medication delivery to clients and monitor adherence to prescribed treatment regimens.Assist clients in renewing or applying for benefits, including Medicaid, Social Security, housing assistance, and other community services.Collaborate with other service providers and natural supports, ensuring clear communication and coordination of services.Actively participate in team meetings and care conferences to discuss client progress and share insights with team plete required documentation in compliance with organizational and regulatory standards.Perform other duties as assigned to support client care and department operations.Promote an inclusive, diverse, equitable, and respectful work environment.QualificationsHas a master's or other graduate degree in behavioral sciences or related fields ORHas a bachelor's degree in one of the behavioral sciences or related fields and has 2,000 hours of experience in the delivery of services to persons with mental illness ORHas 4,000 hours of experience in the delivery of services to persons with mental illness.PreferredOne-year work experience in case management or providing community-based mental health services.Additional RequirementsA valid Class 'D' Driver's License, reliable vehicle, satisfactory driving record and necessary insurance.Must not be actively receiving services from Guild Incorporated and must not have received services within the last two years.Must be available to provide on call crisis support 247 in weekly intervals on rotation.Guild's Employer PromiseWe know you do your best work when you feel supported and have the flexibility to meet all of life's demands, no matter what those demands might be.
