LMSW/LCSW - Social Worker - MIT Harlem Clinic

A health services organization in New York City is currently seeking a new Licensed Master Social Worker (LMSW) andor Licensed Clinical Social Worker (LCSW) to join their Mobile Integration Team (MIT). Responsibilities: The Social Worker (LMSW LCSW) will be responsible for: Providing Support and Clinical services to the assigned clients within the guidelines of Social Work professional Values and Ethics, NYS OMH and MPC Dept. of Social Work Providing outreach and engagement to motivate the clients to be involved in treatment Functioning as resource to provide linkages to community-based resources; completes any referrals necessary for services Providing crisis interventions and ego support services Providing individual or group supportive therapy Maintaining accurate and timely documentation, including 725s, 116s, Individual Service Plan, Service contacts and MHARS documentation Attending daily meetings, discuss pertinent treatment needsoptions, progress with the MTI Team about the assigned clients Qualifications: 1+ year of relevant work experience Active NYS Licensed Master Social Worker (LMSW) andor Licensed Clinical Social Worker (LCSW)
