RN Case Manager - Hospice

They also offer health plans for Medicare and dual-eligible individuals.Their ability to serve at-risk members of the community is supported by the efforts of their non-for-profit foundation, which raises funds to help support their quality programs and services notreimbursed by insurance or government programs. In addition, the generosity of grateful families,private and corporate donors and grants, as well as their own employees, also support theirspecialized services.Responsibilities:In this field-based position, you will visit Hospice and Palliative Care patients to perform bothphysical and psychosocial assessments and electronically document visit occurrences. Patient visitswill be made in both community and facility settings. As the Hospice Field Case Manager, you willdevelop and implement nursing plans of care and make recommendations for care based onpatients' needs.The schedule includes working one weekend per month and functioning in a backup On Callcapacity 2-3 times per month.
