CHS is also a pivotal partner in New York City's criminal justice reform efforts.Job DescriptionPurpose of Position:Under direction of the Gender-Related Services Director, the Case Manager will provide reentry support to all women in RMSC, not already served by discharge planning services. The Case Manager will conduct intake assessments in order to develop individualized transition plans with a focus on continuity of care in the community.The position is located on Rikers Island in the Rose M. Singer Center (RMSC).Examples of Typical Tasks:Meet with assigned patients to introduce services and complete intake assessments.Meet with patients in clinic andor housing area to complete interview and obtain consents. Assist with Medicaid applications and obtaining necessary patient consents as needed.Create individualized transition plans for each patient according to the patient's goals and needs.Tasks may include, but are not limited to: Coordinating medical appointmentsEnsuring medications are called into a pharmacyIdentifying housing resourcesFacilitating referrals to community-based organizations.Conduct follow-up visits with assigned caseload as related to developing a transition plan and reviewing prior to plete documentation in electronic medical records system (CHER); scan and upload relevant consents and documents.Serve as conduit between patient and clinical court advocates; Assist as needed with obtaining consents, relaying information, and other tasks. Attend weekly team rounds and participate in patient case conferences with other discharge planning program staff.Prepare patientscases to discussComplete assignments generated from team discussion; follow-up with supervisorAdditional tasks and projects as requested by Program Manager andor Director of Special Populations (may include, but not limited to: research, developing materials, didacticstrainings). Departmen Preferences:4-year college degree; Preference for degrees related to human service (e.g. psychology, sociology, criminal justice, social work) One or more years' experience providing direct case management services to individualsDemonstrated commitment to care for vulnerable patients Demonstrated career Interest in issues in women's issues, LGBTQI+ issues, health advocacy, compassionate release policy, andor incarcerationFamiliarity with Excel, electronic medical records and other relevant software Prior experience working in jail system strongly preferred; court system or other criminal justice arena consideredMinimum Qualifications1. A four-year high school diploma or its educational equivalent and four (4) years of satisfactory full-time experience providing direct counseling, guidance, crisis-intervention or information and referral services for inmates of a correctional facility, substance abusers or a similar client population; or 2. Education and experience equivalent to "1" above. Thirty (30) semester credits from an accredited college will be considered equivalent to one (1) year of the required experience.
