Serves as a linkage between patients and primary care providers to improve the coordination of care.Collaborates with physicians, clinic staff and other interdisciplinary staff to identify care needs and connect patients with appropriate services.Coordinates care for patients who have gaps in care, or quality flags, to close these gaps by outreaching, engaging and scheduling patients.Coordinates patients scheduling, cancellations, and municates with patients through telephone, mail, MyChart and text messages where appropriate, to increase patient attendance.Answers the telephone in a timely manner, politely and in a professional manner.Meets with patients, primary care and BH providers and other staff as needed to coordinate care.Encourages patient self-management with a focus on the achievement of care plan goals.Documents and maintains patient-specific records.Maintains accurate daily logs for statistical reporting purposes and for weekly reviews of productivity.Meets leadership when necessary to discuss various concerns involving the functions and performance of the Ambulatory Care pletes mandatory hospital in-services via Healthstream.Log in to E-Mail, and any other necessary program daily.Performs any other duties as assigned.Education Required - High school diploma, Bachelors preferredLicenseCertificationRegistration(s) Required - NoneExperienceSkills Required:Minimum of two years' experience in the healthcare industryAdvanced understanding of medical terminology and administration processes.Proficient in information management programs and MS Office.Outstanding communication and interpersonal abilities.Strong attention to detail with excellent organizational skills.Demonstrated proficiency in English and Spanish, preferred.Ability to work effectively in a team.
