Our team is focused on building trust-based engagement and wrap-around support for expectant moms at home, in person and virtually, to ensure they have all the social support they need for a successful and joyful pregnancy journey. We leverage technology to support our virtual engagement that allows us to meet all our moms where they are.We ensure that our members do not feel alone during their pregnancy journey, and that they have a trusted resource for support, resources, and information throughout their pregnancy, delivery and well into postpartum. Cayaba Care does not replace the primary obstetrician or doctor's relationships. Our Maternity Navigators are the anchor of our work; they are hired from the communities we serve, they understand expectant moms and they build deep and meaningful connections to support our members. The MNs build supportive relationships with our members, meeting with them monthly - virtually or in-person depending on the member's need and preference - to provide key support and resources throughout their pregnancy journey. This role will report to the Director of Care Operations.Work Schedule:This role is expected to work the following schedule (paid hourly):Monday, Tuesday, Wednesday, Friday: 8:30am-5pm (hybrid)Thursday: 10am-6pm (hybrid)Saturday: 1x a month 9am-1pm (remote)One week on-call per monthResponsibilities:Manage a panel of members through their prenatal and postpartum journey, connecting with them at least once a month to meet their needs at each stage of pregnancyConduct in-home and virtual visits to provide health education, emotional support, self-advocacy training, and monitor vital signs - empowering members to actively engage in their careIdentify and address Social Determinants of Health (SDoH) such as food insecurity, transportation, or housing challenges, connecting members with community resourcesProvide ongoing support via phone call or messaging between visits and escalate clinical issues to the Cayaba cliniciansCollaborate with the multidisciplinary care team, referring members to Cayaba specialists and sharing updates during daily care team huddles for comprehensive careOffer hands-on, compassionate support as a certified doula, providing emotional and physical assistance during labor, and educating members on birth positions and breathing techniques prior to birthDocument all care interactions using our technology platforms to maintain accurate records of care plans, services provided, referrals, and SDoH screening resultsFollow our Cayaba policies and procedures to ensure the highest standard for our membersQualifications:Minimum of 1-3 years of experience working with patients in a health care setting as a Certified Medical Assistant, Certified Community Health Worker, or DoulaStrong organizational and problem solving skills, with attention to detailAbility to manage multiple priorities such as member visits, messaging, and internal meetings in a fast-paced environmentExcellent communication skills with a strong ability to build supportive relationships and provide compassionate, culturally sensitive care for expectant momsExcitement to work with a collaborative and interdisciplinary team of specialists to create a joyful pregnancy and birth experience for every memberValid driver's license with a clean driving record, reliable transportation, and compliance with state inspection, registration, and insurance requirementsAccess to home internet and a phone with internet accessWhat we're looking for:1-3 years of community work, patient advocacy, or health navigationExperience providing prenatal and postpartum health education (ex. preeclampsia, urgent maternal warning signs, and breastfeeding benefits)Experience in OB-GYN or Women's Health careComfort working with technology platforms and ability to quickly learn new tools and systems such as AthenaOne, Welkin, and MacOsNice to haves but not required:Breastfeeding Counselor certificationFluency in Spanish or FrenchStart-up experience, or adaptability in a fast-evolving environmentAt Cayaba Care, we recognize that great talent deserves great rewards. The hourly rate for this role is between $18 - $28 per hour, depending on skills and experience.
