Operating 24 hours, 7 days a week, we offer reliable, dignified care in your own home and specialize in health services, personal care, and champion services.The Intake Supervisor plays a crucial role in ensuring the efficient processing of patient information and documentation within a home healthcare setting. This position is responsible for overseeing the intake process, managing patient service inquiries, and ensuring that all documentation is accurate and compliant with medical standards. Responsibilities Include: Oversee the intake process for new patients, ensuring all necessary documentation is collected and reviewed. Conduct documentation reviews to ensure compliance with healthcare regulations and standards.Conduct thorough assessments of prospective patients to determine their care needs and eligibility for admission, collaborating with healthcare professionals as neededAssign initial ICD 10 code to initiate authorization process for all referrals for HMO patientsEstablishing & maintaining relationships with local hospitals, physicians & community organizations for the purpose of generating consistent admissions to the facilityUnderstand the admissions process including policy, procedures, assessments & paperwork requirementsCommunicate special needs of new admissions to staff to ensure a smooth transitionWork closely with the CHHA's interdisciplinary team, including nursing staff, social workers, and administrators, to coordinate admissions and provide comprehensive care to patientsPosition Requirements:Degree in Nursing, Social Work or a related fieldLicensed RN or Social Worker strongly preferredPrevious experience in a healthcare admissions or discharge environmentExperience working in a CHHA (Certified Home Health Agency)Previous experience in a management, supervisory or team lead roleKnowledgeable in Home Care billing (Medicare, Medicaid, 3rd party payor)Knowledgeable in ICD-10 medical codingWhat's in it for you:Key clinical supervisory role responsible for making a difference in our patient's lives10% of salary employer contribution retirement plan with a 3-year vesting scheduleGenerous medical, dental, vision, and life insurance coverageEmployer contribution toward medical deductible (effective day 1 of employment)Generous PTO package including holiday, sick, vacation and personal days (including your birthday!)
