Nurse Home Visitor, R.N.Role Overview: The Nurse Home Visitor (R.N.) will provide in-home visits to pregnant and parenting women, as well as their families, who qualify for the Nurse-Family Partnership (NFP) Program. This role focuses on delivering education, case management, and support from pregnancy until the child's second birthday, primarily serving residents of Northern Queens. The nurse will adhere to the nursing process and the NFP home visitation model while maintaining a caseload of at least 25 clients. Key Responsibilities:Home Visitation & Client Support: Establish and maintain therapeutic relationships with clients using reflection and motivational interviewing techniques. Conduct home visits following the NFP model and guidelines. Assess the physical, emotional, social, and environmental needs of clients and their families. Collaborate with clients to set and achieve personal and family goals. Provide education, support, and referrals to assist families in reaching their objectives. Clinical & Case Management Duties: Apply the nursing process to integrate NFP principles into practice. Coordinate with other professionals to ensure comprehensive care for clients. Refer families to appropriate services and conduct follow-ups to confirm service delivery. Complete all necessary documentation for Medicaid billing. Perform additional duties as assigned.
