Every employee takes a person-centered approach that exemplifies the ICARE values (Integrity, Compassion, Accountability, Respect, and Excellence) through empathic communication and partnerships between all persons.Work Shifts9:00 A.M - 5:00 P.MJob Description•Asses and Counsels patients admitted to programs within Chemical Dependency Services, utilizing•Accepted current knowledge-based skills in the treatment of addictions.•Identifies specific problems of patients; is sensitive to cultural and spiritual needs of patients.•Listens to patient's complaints and promptly reports them to immediate supervisor.•Evaluates the progressor lack of progress of patients.•Develops patient treatment plans and establishes short-term and long-term goals in consultation with clinical treatment team.•Participates in clinical team meetings in discussing individual cases and general patient progress.•Develops accurate psychosocial history of patients.•Conducts groups and psychoeducational training sessions with patients.•Maintains accurate, confidential, and legible medical records on each patient according to agency requirements.•Financially screens all patients for admission and completes all admission forms.•Initiates and completes OASAS ADMISSIONDISCHARGE (PAS 46) forms.•Initiates discharge planning and referrals, acting as liaison for patient and completes Discharge Summaries.•Is knowledgeable of Hospital, Departmental and unit Policies and Procedures, and•Demonstrates the ability to perform hisher duties in accordance.•Trains and conducts overdose prevention training and distributes naloxone rescue kits.•Is courteous and professional when interacting with colleagues, patients and the public•Other tasks as determined by department need and the Director of Chemical Dependency Services•Is knowledgeable of hospital and departmental policies and procedures, and demonstrates the ability to perform duties in accordance.Minimum QualificationsAny individual working in a NYS Office of Addiction Services and Supports (OASAS)-Certified Treatment Program or newly-hired into the System in the title of Addiction Counselor in a NYC Health + Hospitals health care setting must have proof of completion of OASAS "Supporting Recovery with Medications for Addiction Treatment (MAT)" training.For Assignment to Level I1. Valid Credentialed Alcoholism and Substance Abuse Counselor (CASAC) - Level 1 Certificate issued by the NYS Office of Addiction Services and Supports (OASAS); or2.
