Job DescriptionJob DescriptionPlans and conducts nutritional programs, under supervision, to assist in the promotion of health and control of disease. Provides nutritional assessment, education and counseling to individuals andor groups. Serves as a resource on nutrition and dietetics to nurses, health educators or other health care providers in the facility.Dept: Ambulatory Care - OB patients3 Shifts: Monday 9am-5pm, Wednesday 9am-12:45pm, Thursday 9am-5pm. Address: 270-05 76th Avenue, Oncology Building, C-Level, New Hyde Park, NY 11040This is a temporary assignment for roughly 3-4 months.Job ResponsibilityPlans and conducts nutritional programs, under supervision, to assist in the promotion of health and control of disease.Assesses nutritional needs and considers patients' cultural, educational, and learning issues.Provides individualized nutrition education to patients, their families, andor significant others based on their needs assessment.Serves as a resource on nutrition and dietetics to nurses, health educators or other health care providers in the municates patient information to referring providers and appropriate staff in a timely manner.Documents nutrition information discussed, written materials provided, and evaluation of patient's receptiveness and comprehension of the education provided.Performs related duties as required. Duties not mentioned here, but considered related are not essential functions.Job QualificationCurrent Diabetes Educator Certification, required.Bachelor's Degree in Food, Nutrition or Dietetics, required.Registered Dietitian, required. ORCurrent Diabetes Educator Certification, required.Bachelor's Degree in Nursing, required.Current license to practice as a Registered Professional Nurse in New York State, required.
