Position Purpose: The Clinical Nurse Coordinator role on the Inpatient Nursing Units is a critical position designed to assist the Nurse ManagerDirector in providing administrative and critical leadership; presents self as a dedicated healthcare professional, patient advocate, mentor, clinical resource, and leader. The Clinical Nurse Coordinator role is essential in improving the patient experience on the units as well as elevating quality of care delivery and patient safety. Under the direction of the Nurse Manager assists with orientation and training for new employees, ensures compliance with all regulatory agencies by enforcing policy and procedures, and improves patient throughput. In the absence or at the direction of the Nurse ManagerDirector, the Clinical Nurse Coordinator will maintain the operational and clinical aspects of the unit. Responsibilities include, but are not limited to assisting with staff development, patient flowthroughput, quality, patient safety, and patient experience. The Clinical Nurse Coordinator is expected to communicate and collaborate with patients, families, physicians, and all other healthcare professionals to meet optimum outcomes. The Clinical Nurse Coordinator works as needed providing direct line care to patients and enters into staffing as needed. This role may be staffed for both day and night shifts and is required to provide clinical support and leadership outside of regular business hours. This position also works rotating weekends and holidays. This position reports to nurse manager and provides support to unit staff. Key Job Activities: Collaborates with and assists Manager with the planning, promotion, and management of operationsservices to ensure quality, efficiency, and cost-effectiveness. Collaborates with leadership team including access team, house supervisor and unit manager with patient movement including bed placement, team assignments.Assists with the implementation of measures to improve quality of care and monitor patient satisfaction. Example (but not limited to); completion of unit audits, such as purposeful hourly rounding, white board completion, Bedside Shift Report (BS), foley, IV line, pressure ulcer and fall risk intervention audits and action planning. Cooperates with the Rehabilitation Team to support the interdisciplinary plan of care. Facilitates RN attendance and participation in team conference and may participate in Team Conference as needed.Maintains a professional demeanor; serves as a clinical leader who leads by example and is a resource and mentor for staff. Promotes positive customer relations for the effective and efficient daily operation of the unit, including customer service recovery. Identifies potential and actual problems in the delivery of patient care, solves problems, and provides solution-based suggestions for implementation and communicates observations to the manager. May participate in interviewing potential nursing staff with the Nurse Manager.Collaborates with Nurse Manager in cost containment strategies, including evaluation of PAR levels and staffing strategies. Partners with key departments including EVS, case management, etc. Utilizes the continuous quality improvement process to assess outcomes and contribute to the design of improved processes. Makes recommendations for patient care to achieve quality and superior service. Participates in QI initiatives such as HAPI surveillance days (four per year) and helps to engage staff in participation of targeted initiatives.Assists with quality monitoring and documentation auditing as indicated for rehabilitation population. Reports Risk Management issues to Nursing Manager and Risk Management (Midas) in a timely manner. Promotes and supports an environment that is conducive to staff growth and development, job satisfaction and autonomy. Practices positive coachingmentoring skills to foster staff growth and development. Facilitates staff involvement in retention and recognition strategies including staff meetings, stop light report, and team building. Participates in the implementation of programs and practices designed to improve morale, employee attitudes, working conditions and communication between all employees. Promotes participation in educational opportunities in partnership with manager and educator.Fosters ongoing professional development of staff by assisting manager with feedback for evaluation, supporting educational programs and may provide education to staff. Promotes an atmosphere of mutual respect between all disciplines. Assists managers with addressing work and minor performance variances through coaching. Maintains confidentiality in matters relating to staff issues. Assumes charge nurse responsibilities as well as provides direct patient care as needed including but not limited to admissions, discharges, transfers, medicine administration and other patient care needs including a patient assignment. Makes rounds on unit to assess status on patients and caregivers and identify needs - documented as directed per policy. Serves as a liaison to patients and families to assist with problem-solving and care coordination. Provides leadership in emergency situations and during disasters or crises situations. Reports need for repair or maintenance of equipment and facilities in the SharePoint site and provides ticket number to the manager and appropriate department. Reported out on safety huddle as needed. May assume responsibilities for basic unit operations in the absence manager. May cross train to house supervisor role for emergency situations. Maintains current standards of care via literature, educational events, internal and external resources, and internalexternal networking. Maintains memberships in professional and community organizations. Participates in activities to enhance professional growth by involvement in professional organizations and continuing education. Performs other duties as assigned. Education and Work Experience: Bachelor of Science in Nursing (BSN) preferred. For acute careICU:3-5 years of clinical nursing (rehab) background are required.Five years of progressive clinical experience include Acute Care Cardiac Nursing, Acute Care Telemetry, Critical CareEmergency Room Nursing, Case Management, Charge NurseHouse Supervision, or Transfer Center Experience. For inpatient rehabilitation:2-5 years of clinical nursing background including rehabilitation nursingPrior progressive clinical leadership experience as a preceptor andor charge nurseKnowledge and Skills: Progressive track record of positive clinical expertise and clinical leadership Experience in EPIC and MEDITECH software systems preferred. CertificationsLicenses: Registered Nurse (RN) with valid Illinois licenseBasic Life Support (BLS) requiredCRRN preferred and required within 3 years of assuming positionWorkplace Conditions: PHYSICAL DEMANDS: While performing the duties of this job, the employee is regularly required to sit; use hands to finger, handle or feel; and talk or hear The employee frequently is required to walk and stoop, kneel, crouch The employee is occasionally required to stand The employee must occasionally lift andor move up to 25 pounds.
