This includes but not limited to the outcomes related to nursing sensitive indicators. The clinical nurse manager leads the care team and develops an overall view of patients, families, and caregivers, and establishes strong partnerships with them to ensure the delivery of timely and optimal care. The clinical nurse manager is highly visible and works closely with all members of the team. Ensures ongoing compliance with accreditation, regulatory, and professional standards impacting the clinical area.These duties are to be performed in a highly confidential manner, in accordance with the mission, values and behaviors of Insight Chicago Hospital and Medical Center, 14 Forces of Magnet and the attributes of Careful Nursing under the supervision of the area director. Employees are further expected to provide a high quality of care, service, and kindness toward all patients, staff, physicians, volunteers and guests.DESCRIPTIONEssential Duties and Responsibilities1. Serve as a resource and mentor in decisions for clinical interventions in collaboration with the interdisciplinary healthcare team. Is highly visible and accessible to the team Ensures the coordination of care for a group of patients, identifying interventions based on desired outcomes consistent with evidence-based guidelines and quality care standards. Participate in establishing and reviewing interdisciplinary patient care plans with team members using patient information system data when appropriate Analyze unit resources and set priorities for maximizing outcomes. Identify clinical and cost outcomes that improve safety, effectiveness, timeliness, efficiency quality and client-centered care. Establishes unit based quality indicators and participates in hospital wide quality, research and safety initiatives Promotes nursing process, by ensuring clinical competence 40%2. Responsible and accountable for ensuring continuity of care for patients. Ensures communication, provides support to nurses, CMs and SWs, pharmacist, navigators, community and crisis workers as well as any other health care professionals who provide care to the same clients across patient care settings Oversees the coordination and evaluation for plans of care Ensures multidisciplinary team huddles incorporating MD, patient and family as part of the meeting. Assist team in setting patient daily goals Analyze interdisciplinary patterns of communication and chain of command that impact care both internal and external to the unit. Empower staff to improve operations, outcomes and encourage participation in shared decision making Works with staff to promote recruitment and retention initiatives and embrace a positive work environment and employee relations 20%3. Provide leadership for implementation of evidence-based practice. Apply evidence-based practice as a basis for patient care decisions. Review and implement patient care guidelines and protocols to address patient care issues. Discover, disseminate and apply evidence for changing practice. Participate in development of or changes in policies and procedures related to patient care. Mentors professional nursing staff related to practice and outcomes Provides formal feedback with the annual evaluation process for nursing practice and competency Enhances employee satisfaction with timely feedback, holds regularly scheduled meetings and is available for all shifts Ensures staff availability via schedules and oversight of time and attendance 10%4. Leverage available technology to provide useful and easily accessible information regarding patient outcomes. Present to the multidisciplinary team cost saving ideas that improve patient care outcomes and efficiency. Use aggregate data sets to prepare reports and justify needs for selected care improvements. Utilizes tools to document care appropriately for accurate FIM scores Evaluate the impact of new technologies on healthcare team members, patients and families. Revise patient care based on analysis of outcomes and evidence-based knowledge. 10%5. Use appropriate teachinglearning strategies as well as current information, materials and technologies to facilitate the learning of individual patients, patient groups and other health care professionals. Promotes staff participation to present case studies at grand rounds and team meetings Ensures an education plan for unit-specific patient populations. Engage staff to create education materials for patients and staff, including self-management guides for patients and families. Mentors professional education sessions for the members of the multidisciplinary team based on evaluation of the role of the team, patient interactions, and expected and actual outcomes. 10%6.
