The apartments we provide support in are warm and inviting, with one to three people sharing a home. Each person has their own bedroom and is empowered to make choices about their lives. We are currently seeking a Coordinator in our Supported Living program who will lead a team of Direct Support Professionals to provide support that centers the needs, expectations and individual differences of the people we support, acting with care, dignity, and compassion. A strong candidate for this role will bring strong leadership skills, with an emphasis on critical thinking, clear communication, and follow through. Be sensitive to the needs, expectations and individual differences of the people we support, acting with care, dignity, and compassion.Job Path operates under OPWDD and thus must ensure compliance with their regulations. This is a full-time position at 35-40 hours per week, those hours may include some evenings and weekends. A team of six rotates evening and weekend on call responsibilities.Responsibilities of the Coordinator in Supported Living:Centering the People we SupportFoster open and supportive communication between the people we support, families, and other community organizations and agencies.Coordinateattend meetings related to the people we support (Life Plans, Residential Plan reviews), ensure that next steps are appropriately delegated or followed through onImplement and follow-up on all support services recommendations.Act as liaison for involvement in support groups, self-advocacy, vocational programming, transportation services, integrated community activities, recreation, and learning independent living skills.Work closely with the Medical Coordinator and the Registered Nurse to review medical documentation and ensure all medical follow ups are being done in a timely manner.Provide and ensure communication of medical and health issues to staff, agency nurses, families, etc. using appropriate logs when necessary.Supervising a TeamCreate and distribute the weekly staffing schedule, ensuring that all activities are included and that appropriate levels of coverage are maintained. This includes providing coverage as needed.Schedule formal supervision time with staff on a regular basis, at least monthly, to cultivate a working relationship where team members can discuss challenges and opportunities that they are facing, and constructive feedback can be delivered and received.Train staff and support their development through teaching, acting as a role model, and setting up shadowing shifts with strong DSPs. This includes guiding staff on how to appropriately work with and interact with the people we support, other staff, families and plete annual evaluations for staff and work with them to review plans for professional development in partnership with the Human Resources teamFollow through with ensuring that for Direct Support Professionals complete required trainings, in partnership with the Training and DEIAB CoordinatorParticipate in interviews to screen new staffCommunicate and implement agency policies and procedures.Ensuring a safe physical environmentSchedule and conduct fire and emergency drills with staff and people who we support, making sure to log the drillsPossess a strong working knowledge of all required emergency procedures in order to provide leadership during practice and actual drills.Maintain and track inventory of PPE in each apartmentComplete physical plant review and follow up with all repairs as needed. Billing and Compliance DutiesCreate schedule blocks in the electronic health records system, being mindful to maximize opportunities to bill appropriate programsReview and approve casenotes submitted by staff and follow up to ensure timely completionEnsure completionsubmission of all reports as scheduled.
