Care Transitions ManagerHealthCare Support is actively seeking a Care Transitions Manager to fill an opening with a prestigious Healthcare companyin Queens.Daily Responsibilities for Care Transitions Manager: Oversees care transitions from hospital to home, ensuring smooth discharges, participant education, and coordination with interdisciplinary teams to improve health outcomes and reduce readmissions.
