Nurse Coordinator

Job DescriptionThe Nurse Coordinator (RN) assists in providing nursing services, education to staff and clients and is a vital member of the ACT Team. The Nurse Coordinator is part of the interdisciplinary team and serves as the expert in assessment, identification of needs and coordination of care and services to facilitate the patient's progression through the continuum of care in a timely and cost-effective manner. Nurse Coordinator duties include care planning, coordination, and assessment of health, environmental and psychosocial status members on an ongoing basis. Nurse Coordinator works alongside the Psychiatrist to ensure clients care. Additional Requirements: Minimum of two years of experience in specialty or area of practice, e.g. acute care, ambulatory care, long term care, behavioral health. Basic Life Support required.Physical Requirements: Some lifting of medical equipment and supplies. Prolong hours of sitting andor standing. Travel to and from ACT patients' home and walking up and down stairs. Ability to travel to ACT patients home and conduct nursing and behavioral clinical visits. Able to carry medications to ACT patients home and back to ACT office. Education Requirements: NYS Professional RN license required; Bachelor's degree required; Master degree preferredValid driver's license in good standingProficient of Microsoft Office Suite (Outlook, WORD, Excel, PowerPoint), database navigation required and Electronic Health records navigation required.
