A healthcare organization in New York City is currently seeking a Cantonese, Mandarin, andor Spanish-speaking Licensed Clinical Social Worker (LCSW) andor Licensed Master Social Worker (LMSW) to join their team in the Bronx as a Social Work Care Manager. In this role, the Social Work Care Manager will be responsible for providing psychosocial support counseling and other social work related services to Palliative Care patients and their families in both community homecare and facility settings.
