Job DescriptionJob DescriptionPhysical Therapist (PT) - Coldwater, MI.Job Type: Full-Time.Shifts: Monday through Friday from 7:30 AM to 04:00 PMSummary:ProTouch Staffing has partnered with an established Occupational Health client located in Coldwater, MI that is seeking a qualified Physical Therapist for one of their processing facilities. This is a full-time direct-hire opportunity for candidates that have at least 1 to 2 years of experience and are looking to step into an outpatient role as a Physical Therapist. The top priority of the Physical Therapist is to perform physical therapy evaluation and create personalized treatment plans for team members with injuries or illnesses, supervise physical therapist assistants (PTA)when appropriate, and complete HPEs. The Physical Therapist will also be asked to perform administrative tasks and scheduling to ensure efficient and effective operations of the physical therapy clinic.Under direct supervision of the Clinical Director, the Physical Therapist works within the partner company guidelines, practice standards for their state and professional organization, and our company's policies and procedures.Daily Responsibilities: 1. Physical Therapy Initial Evaluation for workers compensation and/or Direct Access2. Physical Therapy TreatmentDocumentation of treatment sessions on treatment cardsDevelop HEP for patients, using HEP2GO website and print copies for patientsDocumentation of weekly progress note for each patient in OHM3. Email weekly progress notes for each patient to PMA and Case Manager4. Complete OHU form whenever patient status changes5. Scan all OHU forms into the Infirmary Drive6. At physical therapy discharge:Scan Treatment Card to Infirmary driveSubsequently attach all Treatment Cards to appropriate encounter on OHMBring hard copies of Treatment Card and OHU forms to First Aid to be filedSelf-Schedule future appointments for physical therapy initial evaluations, treatments and direct access follow ups.Communicate with supervisors via phone and in-person about patient restrictionsCollaborate with ATC and Case Manager about Plan of Care (POC) for patientsComplete Human Performance Evaluations (once HPE training is completed.)Weekly ResponsibilitesComplete physical therapy statisticContact clinic manager about needed suppliesWeekly case-management meeting to review each patient with staffCall referring providers about protocol and POC (when referred by specialists)Occasional ResponsibilitesTrigger Staff/Temporary staff onboardingTrigger staff billing/SuperBillConsult for ergonomic assessment on the floorCreate onboarding/SOPsQualifications:Doctor of Physical Therapy (DPT) or Master's in Physical Therapy (MPT) from an accredited program.1-2 years of experience as a Physical Therapist.Current Michigan state licensure as a Physical Therapist.Current BLS certification.Benefits:Dental, health, vision, CME, PTO, etc. - These will be discussed in more detail during your interview.To proceed with an application please upload your resume to the job portal and we will get back to you within 1-2 business days.
